RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT

Considerations to participate in any sports activity of GSAI.

1. I acknowledge and agree that I am qualified, medically able, in good health, and in proper physical condition to participate.

2. I fully understand the risks and dangers of serious bodily injury or other adverse health consequences or loss/damage of property and I fully assume all risk and responsibility.

3. I do not have COVID-19 or other infectious diseases.

4. I hereby agree to wear the proper sports gadgets, and all required protective gear.

5. I am accountable to any sports equipment that I will be using is in good working and safe condition.

6. I will follow the rules and regulations that GSAI are implementing.

7. I agree that I am solely responsible for my compliance with all local laws.

I have carefully read this, and I fully understand its terms and conditions.

I understand that I am responsible for monitoring the condition of myself and I agree to immediately stop participating if I or any other members that I know, experiences unusual symptoms, issues or conditions, including but not limited to medical or health symptoms.

As a member, I agree to disclose to GSAI any illness or injury that may have a negative effect on my fellow members should I participate in any Sports. All relevant personal health and fitness information both prior to and during engagement in the Sports or events should be made known. I must be fit enough to undertake the sport I have entered and not present a danger to myself or others through undisclosed illness or injury.
