I understand that payments must be made, confirmed, and fully submitted as Proof of Payment to have my Registration be valid and can be used to reserve and claim the Bike Ride T-shirt (non-replaceable).

I understand that claiming the t-shirt will be First Come - First Serve for the first 1,000 t-shirts produced (100 small, 300 medium, 300 large, and 300 extra-large), based on availability. That t-shirt outside the range will be ordered separately. Payments received and confirmed before April 15, 2021, and the size required is out of stock, this will be ordered, and pick-up might be available before May 1st, 2021 (depending on current market/IATF restrictions). Payments received from April 16 to April 30, 2021, t-shirt will be ordered but pick-up might be after May 1st, 2021.

I understand that there are 2,000 booklets produced and distributed. In case there are requirements for more booklets, the organizer can receive firm payment until April 24, 2021, and the booklets will be ordered, produced, and distributed before May 1st raffle draw.

I understand that if the payment were given but the registration form was not submitted before May 1st, 2021, no t-shirt was allocated, and the booklet buyer forfeits the t-shirt claim.

I understand and agree that by submitting this form that I release the organizers entirely free from all liability and any financial responsibility relating to any injuries or accidents that may occur during the events proceeding, relating to, sprain, fracture, neuromusculoskeletal injuries, and other physical and mental injuries regardless of whether these are caused by negligence. I swear that I am participating voluntarily and that it comes with risks. Additionally, I do not have any conditions, illness, and diseases that will increase my likelihood of experiencing injuries to myself and others while engaging in this activity and I forfeit any right, claim, or liability to hold organizers responsible for any tangible or intangible damages, loss of property, or loss of life may occur during or after Fiesta Family Bike Fun Ride related activities. I will also make every effort to obey safety precautions.

It is my responsibility to consult a physician before participating in this program and I affirm that I have no medical conditions that would restrict me from joining the event and that if I do experience medical issues, I will contact my doctor immediately.

I understand that the organizers reserve the right to cancel the Fiesta Family Bike Fun Ride for reasons attributable to Covid-19 based on LGU or IATF Restrictions or any other reasons out of the hands of the organizer. If the event must be canceled, organizers have no obligation to refund the registration fee, pay damages, or any other compensation. I agree that the registration is non-refundable. The organizers will continue the raffle draw on May 1st, 2021.

I authorize the organizers to utilize my photos and videos as well as any media, photos, or footage from the event solely for the purpose of marketing.
